Assessing the progress of integrating health and social services in Wales, with a particular focus on the delivery of care for older people, this seminar was an opportunity to discuss progress in implementing the Welsh Government's Framework for Delivering Integrated Health and Social Care for Older People with Complex Needs, introduced in April 2014 - which aims to ensure services, care and support are designed, co-ordinated and delivered effectively and meet the needs of patients. Delegates also assessed the impact of the Intermediate Care Fund since its introduction and the next steps to ensure further collaboration between services.
Further sessions discussed the potential benefits and barriers of moving towards an integrated service - including co-production between services, establishing a new standard model of care across Wales, strengthening community based care, dependency prevention and preventative intervention measures, as well as the changing role of secondary services.
The conference brought together key policymakers and regulators with stakeholders including NHS staff, Local Health Board representatives, community health councils, patient representatives, academics and other interested parties.
